Compassion Care (doing business as Heal+Glow Wellness) may revise and update our Privacy Policy and Disclaimers at any time. Compassion Care is dedicated to maintaining the privacy of your identifiable health information. In conducting our business, we will create records regarding you and the treatment and services we provide to you. We are required by law to:

Treatment
Every appointment you make with Compassion Care is recorded. Usually, this record contains your symptoms, examination and test results, diagnoses, treatment and a plan for future care or treatment. This is referred to as your medical record.
We may use your identifiable health information to:
- Plan your medical care and treatment.
- Communicate with other health professionals who may contribute to your care.
- Provide to you or a third-party payer documentation that services billed were actually provided.
We may disclose your identifiable health information to pharmacies, physicians, nurses, and other healthcare personnel who may use or disclose this information to treat you or to assist others in your treatment. Additionally, we may disclose your identifiable health information to others who may assist in your care, including but not limited to your spouse, children, or parents.
Payment
Compassion Care may use and disclose your identifiable health information to bill and collect payment for the services and items you may receive from us. For example, we may contact your health insurer to certify that you are eligible for benefits, and we may provide your insurer with details regarding your treatment to determine if your insurer will cover or pay for your treatment. We may also use and disclose your identifiable health information to obtain payment from third parties that may be responsible for these costs, including but not limited to family members. We may also use your identifiable health information to bill you directly for services and items.
Healthcare Operations
Compassion Care may use and disclose your identifiable health information in order to operate our business. These uses and disclosures are necessary for our business activities and to make sure our patients are receiving quality services. As examples, our practice may use your identifiable health information to evaluate the quality of care you receive from us, or to conduct cost management and business planning activities for our practice.
Health Information Exchanges
We may access, use and disclose your health information via electronic queries and both to and from other providers who have a relationship to your treatment. Examples of this include, but are not limited to, the electronic query functionality of Optimantra. If you do not want to have your health information shared with the electronic health exchanges we participate in, you can contact us to opt out.
Business Associates or Consultants
Compassion Care may contract with other organizations or people, known as business associates or consultants, to assist us in performing services or operating our business. Your health information may be shared with our business associates or consultants only if they have agreed to protect your information in the same manner as Compassion Care.
Required by Law
Compassion Care will use and disclose your identifiable health information when we are required to do so by federal, state or local law.
Public Health Risks
Compassion Care may disclose your identifiable health information to public health authorities who are authorized by law to collect information to prevent or control disease, injury or disability, report births and deaths, report child abuse or neglect and notify a person who may be at risk for contracting or spreading a disease or condition.
Health Oversight Activities
Compassion Care may disclose your identifiable health information to a health oversight agency for activities authorized by law. These can include investigations, inspections, audits, surveys, licensure and disciplinary actions; civil, administrative and criminal procedures or actions; or other activities necessary for the government to monitor governmental programs, compliance with civil rights laws and healthcare systems in general.
Lawsuits and Similar Proceedings
Compassion Care may use and disclose your identifiable health information in response to a court or administrative order if you are involved in a lawsuit or similar proceeding. We may also disclose your identifiable health information in response to a discovery request, subpoena or other lawful process by another party involved in the dispute; but only if we have made an effort to inform you of the request or to obtain an order protecting the information the party has requested.
Law Enforcement
Compassion Care may disclose identifiable health information for law enforcement purposes as required by law or in response to a valid subpoena.
Deceased Patients
Compassion Care may release identifiable health information to a medical examiner or coroner to identify a deceased individual or to identify the cause of death. If necessary, we may also release information to funeral directors as necessary in order for them to carry out their duties.
Organ and Tissue Donations
Compassion Care may release your identifiable health information to organizations that handle organ, eye or tissue procurement for transplantation, including organ donation banks, as necessary to facilitate organ or tissue donation and transplantation if you are an organ donor.
Research
Compassion Care may use and disclose your identifiable health information for research purposes in certain limited circumstances. There are times when your health information may be reviewed by research staff or a clinic provider if it is believed you may be interested in or benefit from a research study. Your identifiable health information will not be disclosed outside of Compassion Care for research purposes without your written permission or making sure your privacy is protected. Health information about you that has had identifying information removed may be used for research without your consent.
Health-Related Services
Compassion Care may use your health information to contact you with information about upcoming appointments, health-related benefits and services, and treatment alternatives, including research studies, that may be of interest to you.
Serious Threats to Health or Safety
Compassion Care may use and disclose your identifiable health information when necessary to reduce or prevent a serious threat to your health and safety or the health and safety of another individual or the public. Under these circumstances, we will only make disclosure to a person or organization able to help prevent the threat.
Military
Compassion Care may disclose your identifiable health information if you are a member of U.S. or foreign military forces (including veterans) if required by the appropriate military command authorities.
National Security
Compassion Care may disclose your identifiable health information to federal officials for intelligence and national security activities authorized by law. We may also disclose your identifiable health information to federal officials in order to protect the President, other officials or foreign heads of state, or to conduct investigations.
Inmates
Compassion Care may disclose your identifiable health information to correctional institutions or law enforcement officials if you are an inmate or under the custody of a law enforcement official. Disclosure for these purposes would be necessary, (a) for the institution to provide health care services to you, (b) for the safety and security of the institution, and/or, (c) to protect your health and safety or the health and safety of other individuals.
Workers’ Compensation
Compassion Care may disclose your identifiable health information to comply with laws relating to workers’ compensation or similar programs.
Health Insurance Portability & Accountability Act (HIPPA)
Please find below the Health Insurance Portability and Accountability Act (HIPAA), dealing with the privacy and security of health information. HIPAA applies to all staff of Compassion Care, students, interns and consultants.
This notice describes how health information about you may be used and disclosed and how you can get access to this information. Please review it carefully. You have the following rights regarding health information we maintain about you and your family:
Right to Inspect and Copy
You have the right to inspect and copy health information that may be used to make decisions about your care. Usually, this includes medical and billing records, but does not include psychotherapy notes, information compiled in reasonable anticipation of, or for use in, civil, criminal, or administrative proceedings, or psychological test materials.
To inspect and copy health information, contact our office. If you request a copy of the information, we may charge a fee for the cost of copying, mailing, or other supplies associated with your request.
We may deny your request to inspect and copy in certain very limited circumstances. If you are denied access to health information, you may request that the denial be reviewed.
Right to Amend
If you feel that health information we have on file is incorrect or incomplete, you may ask us to amend the information. You have the right to request an amendment for as long as the information is kept by or for Compassion Care.
To request an amendment, your request must be made in writing. In addition, you must provide a reason that supports your request. We may deny your request for an amendment if it is not in writing or does not include a reason to support the request. In addition, we may deny your request if you ask us to amend information that:
· Was not created by us, unless the person or entity that created the information is no longer available to make the amendment
· Is not part of the health information kept by or for Compassion Care
· Is not part of the information which you would be permitted to inspect and copy
· Is accurate and complete
Right to an Accounting of Disclosures
It is very rare that we would disclose health information except for purposes of treatment, payment, or operations. However, if we do make other disclosures, you have a right to request an accounting of these disclosures.
To request this list or accounting of disclosures you must submit your request in writing. Your request must state a time period which may not be longer than 3 years. The first list you request within a 12-month period will be free. For additional lists, we may charge you for the cost of providing the list. We will notify you of the cost involved and you may choose to withdraw or modify your request at that time before any costs are incurred.
Right to Request Restrictions
You have the right to request a restriction or limitation on the health information we use or disclose for treatment, payment, or operations. You also have the right to request a limit on the health information we disclose to someone who is involved in your care or the payment for your care. Please note, we are not required to agree to your request. To request restrictions, you must make your request in writing. In your request you must tell us:

Right to Request Confidential Communications

We have general health and wellness information on our website. Visitor Data
Compassion Care maintains permission to gather information on basic usage, such as numbers of visits, visitor source and visitor activity. This data is collected for the purposes of improving the overall user experience. This information is not shared with other organizations for their independent use.
Password protection
Access to our patient portal is password protected. We strongly recommend that you do not divulge your password to anyone and that you update your credentials on a regular basis. Exercise the utmost personal security in how you handle your password. Compassion Care will never ask for your password in an unsolicited phone call or unsolicited email.
Emails
Your email address is never used for any purpose other than to communicate with you or on your behalf. Your email may also be used for forgotten password or user account information, and in the future, it may be used as a tool for messaging. Unless you opt out, we may also use your email address to send you promotional and marketing materials. Compassion Care never shares your email address with external companies. Calendar
You may give consent/authorization to connect your 3rd party calendars (Office, Apple, Google) with the Compassion Care scheduling module. Compassion Care will read your calendars and display the information in the scheduling section and may cache the information for improved performance. If you provide the appropriate consent/ authorization for adding the Compassion Care calendar to your 3rd party calendars, then Compassion Care will create/modify/delete the details on your 3rd party calendar. Compassion Care will not use any information from your 3rd party calendars for any purposes other than to display the information on the scheduler.
Surveys, Questionnaires & Polls
Compassion Care may ask you to participate in use surveys, questionnaires, or polls, to facilitate feedback and input from our users. When you respond to surveys, questionnaires or polls related to our site, this information is collected only as anonymous, aggregated information and is used for statistical purposes.
Marketing Products & Services
Unless you opt out, we may email marketing materials to you. We may place advertisements concerning the products and services of 3rd parties on our website. We may receive remuneration from the suppliers of these products and services for placing their advertisements. We may use computerized processes to tailor the advertisements to you or to your use of the system. However, except as described above, we will not disclose to any 3rd party any information that identifies you to enable the 3rd party to market products or services to you directly.
